I know the phrase on the observe is American but I can't find an example with a reverse like this?

It's a "love token".
Look it up.
Each one is usually hand engraved and unique.

Maybe her initials had an E, H, and P? What's the size/dia? It looks like a Morgan dollar. Most love tokens that I've seen ar dimes or quarters.

Agree,
It is what is known as a “ love token” on a Morgan Silver Dollar.
Very nicely done by a person with skill. There is a collectors market for some of these.
