2011 Mint error or post mint damage?

This coin has three areas of interest on the reverse side and one prominent spot on the obverse and I was wondering if it could have happened in the minting process or if it is post mint damage? I would appreciate your thoughts/comments. Thanks

PMD. That could not happen during the striking of a coin.
