Sorry but that is not an error, it is damage.
There is no way that could happen during the striking of the coin.

I agree with coinguy. This shape cannot be produced by the Mint.

I agree, not mint. I think that a mallet, pieces of leather and the round end of a ball-peen hammer got some use
